While health care can bring up a lot of questions, protecting your privacy shouldn't be one of them. NSU Health is committed to safeguarding your health information.

You or your personal representative may inspect and/or obtain a copy of your health records or have copies of your health records sent to another health care facility. You or your personal representative will be required to complete and sign an Authorization for Use and Disclosure of Protected Health Information Form before the NSU Health releases your health information.

How to Start a Request for Your Health Records

Obtaining a copy of your health records, or having your health record sent to another health care facility, is easy. To start your request, simply download, print, complete and sign the applicable NSU Health Authorization for Use and Disclosure of Protected Health Information Form (“Authorization Form”) listed below.

Please be sure to complete all sections on the Authorization Form, including your signature. Unsigned requests will not be processed, and failure to provide all the information may invalidate the request. Our Health Records Department staff will be happy to assist you with any questions you may have regarding the Authorization Form. The telephone number for each NSU Health Records Department is listed below for your convenience.

Drop off

Drop off your form at any NSU Health registration desk. Include a copy of a valid photo ID (passport, driver’s license or state ID) — required before processing.

Mail

Mail your form to the NSU Health’ Health Records Department listed below. Include a copy of a valid photo ID (passport, driver’s license or state ID) — required before processing.

Fax

Request for Health Records – Frequently Asked Questions (FAQs)

Information electronically downloaded through the NSU patient portal is provided at no cost. If the health records are being released directly to you, your health care provider, or to another health care facility, there will be no charge associated with copying the records.

For patients only, there is a flat fee of $6.50 for health records requested on an encrypted USB flash drive. All other fees for health records comply with State of Florida statutes and applicable federal regulations.

Yes, you can review your records even if you do not wish to get copies at that time. You would contact the Health Record Department of the applicable NSU Health to schedule an on-site appointment. Please keep in mind the Health Records staff would not be able to answer clinical questions. Should you have clinical questions, please contact your NSU Health provider.

No, there is no fee to the patient or the other entity when records are being sent for continued patient care. However, there would be associated fees for records being requested for example attorneys, and disability insurance companies.

Please note, health records must be completed by all health care providers involved in the patient’s care prior to being copied and released to the patient. This process can take up to thirty (30) days. However, our goal is to complete requests for health records within 10 – 15 business days after receiving the signed Authorization Form. However, records needed for medical emergencies may be faxed directly to a health care provider or medical facility.

We do not fax health records, except in an emergency to another health care provider.

The person who, under Florida law and, consistent with NSU’s HIPAA Privacy Policies, has the authority to act on behalf of the patient in making decisions related to health care provided to the patient (e.g., parent of a minor, a court-appointed guardian). 45 C.F.R. § 164.502(g) (2).
